The first link on your right is for the student to fill out and should be returned to the teacher or counselor that nominated you.

The second link will provide parents with more information about Camp Shaver and program your child will be participating in. You will need to fill out form A B C. Theses should be returned during the pizza party or when you bring your kid to the bus.
